Salvage therapy (voor beide schema's): alleen na 2 weken IV behandeling:

  • linezolid 2x600mg po + rifampicine 2x600mg po OF
  • moxifloxacine 1x400mg po + rifampicine 2x600mg po OF
  • moxifloxacine 1x400mg po + clindamycine 3x600mg po
