caspofungine iv loading dose 70mg, 50mg 1dd

or

caspofungine iv loading dose 70mg, 70mg 1dd
Remarks:

onderhouds dosis 50mg als <80Kg; 70mg als > 80Kg.

General comments

Empirische therapie = therapie bij vermoeden op candidemie.

ALLEEN toe te passen op de ICU, vanaf IC-opnamedag 7, als voldoend aan: 

  1. onverklaarde sepsis EN
  2. significante colonisatie met candida EN
  3. klinische risicofactoren voor candidemie (bv abdominale chirurgie, naadlekkage, breedspectrum antibiotica, centrale lijn in situ)
