General comments
Stop zo mogelijk verantwoordelijk antibioticum, en isoleer patiënt.
Bij 1e relapse/recidief: herhaal behandeling. Multipele recidieven: afbouwschema vancomycine: eerste week na therapie 125 mg 3 dd, tweede week 125 mg 2 dd, derde week 125 mg 1 dd, vervolgens nog 1-2 weken tweemaal per week vancomycine 250-500 mg po.
Bij ernstige infectie overleg met MDL-arts en apotheker over vancomycine via neusmaagsonde en/of klysma
Gebruik ZAR-score voor inschatten ernst infectie (ZAR ≥2 = ernstig): Bij ernstige infectie altijd 14 dagen behandelen.
1 PUNT:
- leeftijd > 60jr
- temp > 38.3
- albumine <25g/L
- WBC > 15x10^9
2 PUNTEN:
- endoscopische aanwijzingen voor pseudomembraneuze colitis
- behandeling in de ICU
NB:
1. Voor de orale behandeling worden vancomycine 125mg capsules gebruikt. Indien er tekort is aan capsules kan uitgeweken naar vancomycine ampullen, oorspronkelijk bedoeld voor infusie. Orale inname van deze vloeistof is veilig en common practice.
2. Orale vancomycine voor poliklinische behandeling is alleen in de botica van HOH verkrijgbaar.
