General comments
Rifampicine pas starten na 3-5 dagen adequate antibiotische behandeling.
Alleen voor kunstklep endocarditis: als aminoglycoside resistent: overweeg het toevoegen van een fluorquinolon aan vanco/rifamp.
Laagdrempelig chirurgische behandeling bij kunstklep endocarditis door MRSA gezien de hoge mortaliteit bij medische behandeling alleen, in het bijzonder als de verwekker aminoglycoside resistent is.

Bij BMI > 30 gentamicine op Adjusted Body Weight (ABW) doseren
