General comments
Algemeen: laagdrempelig overleg cardiothoracaal centrum.
Fluconazol:
-in community acquired oesophagus perforatie, voeg ALLEEN fluconazol toe in éen van de volgende gevallen: langdurig PPI gebruik of immunosuppressiva, HIV met CD4<200, niet opknappen na 3 dagen adequate antibiotische therapie.
-In nosocomiale oesophagus perforatie, voeg altijd fluconazol toe.
Vancomycine dosering: zie hoofdstuk 'overig' --> 'vancomycine continu dosering'
