General comments
Opmerkingen:
Behandeling altijd door of in overleg met orthopeed.
Voor start AB altijd 2x bloedkweek, ook al is er geen koorts
Zo snel mogelijk gewrichtspunctie verrichten op leucocyten, kweek + gram-preparaat en jichtkristallen.
Als jichtkristallen positief: ook antibiotisch behandelen tot kweken negatief.
Indien kweek negatief en niet reagerend op empirische therapie
- herhaal punctie of doe operatieve debridement
- denk ook aan gonokokken (PCR op synoviaal vocht) of inflammatoire arthritis (jicht, RA of reactieve arthritis)
