General comments

  • definities:
    • Sepsis  = infectie + MEWS 5 of hoger.  
    • Septische shock = sepsis met MAP < 65 en lactaat > 2 ondanks adequate vulling
  • Gentamicine: 5-7mg/kg, dagelijks evalueren. 
    • Bij gewicht > 100Kg gentamicine op Adjusted Body Weight (ABW) doseren:
      • 100 - 120 kg: 480 mg 1x per dag
      • 120 - 140 kg: 560 mg 1x per dag
      • 140 - 160 kg: 600 mg 1x per dag
      • 160 - 180 kg: 680 mg 1x per dag
      • 180 - 200 kg: 760 mg 1x per dag
      • 200 - 220 kg: 800 mg 1x per dag
  • vancomycine dosering: zie 'overig' --> 'vancomycine continu dosering'
