	Medication: linezolid po 600mg 2dd + rifampicine po 600mg 2dd	Remarks: Salvage therapy, na ten minste 2 weken IV behandeling.

≥ 18 years

Indications: Penicillineallergie of amoxicilline resistent en low level gentamicine resistent (MIC<=128 mg/L)

	Medication: vancomycine iv continuous per 24 hours 6 weeks + gentamicine iv 3mg/kg dd 4 to 6 weeks	Remarks: natieve klep of kunstklep

≥ 18 years

Indications: Penicillineallergie of amoxicilline resistent en high level gentamicine resistent (MIC>128 mg/L)

streefwaarden vancomycine: 20-25mg/l
